Missionary Flights International

Thank you for your interest in affiliation with Missionary Flights International.  Part of the requirement for affiliation is the completion of this application.  Missions applying for affiliation with MFI should meet the requirement of being a registered Non-profit 501C(3) organization or church.  The mission’s Statement of Faith along with any mission literature should be submitted along with this completed application.  If the mission is approved for affiliation with MFI, then mission personnel may be approved as long as they are working with the approved mission.  They MUST complete a separate application.  Before completing the mission application below, please take a moment to examine MFI's statements of faith and policy, and affiliation procedures and services.  After doing so, please print out and complete this application page, and mail to MFI at the address listed below.  If your organization already has a website, and the site includes the mission's Statement of Faith, you may cut, paste and e-mail back to us this affiliation application.  Please also include a link to the Mission's website address in the space provided.

The affiliation fee is $150 per year and may be paid annually.  The application must be completed and returned to MFI.  If sending by regular mail, please also include your mission’s Statement of Faith, and any mission literature, along with this completed application.
